Vietinis PUVA terapija gydoma tik psoraleeno poveikiu paveiktoms odos vietoms. Mažesni plotai yra tepami su gliukozės turinčiu psoralenu, kuris prasiskverbia po plėvele. Alternatyva yra vonia PUVA, kurioje "Psoralen" kaupiasi šilto vandens vonioje, o maudosi odoje.

Sisteminė PUVA terapija

Sisteminėje PUVA terapijoje psoralenas yra paskirstomas visame kūne. Šiuo tikslu pacientas vartoja psoraleno tabletes dvi valandas prieš UV-A spinduliavimą.

Kada tu darai PUVA?

PUVA yra viena iš efektyviausių šviesos terapijos formų. Atopinė egzema, psoriazė ir kerpių planas yra ypač jautrūs kremai ar vonios PUVA. Sisteminė PUVA daugiausia naudojama lėtinėje odos T ląstelių limfomos ir baltos dėmės ligos (vitiligo). Šios šviesos terapijos formos gali būti naudingos ir kitiems dermatologiniams ligoms. Tačiau spuogai nėra vienas iš jų, nepaisant priešingų nuomonių.

Ką tu darai su PUVA?

Prieš faktinį PUVA terapiją gydytojas neleidžia padidinti jautrumo šviesai vaistų ar piktybinių odos navikų. Jis taip pat nustato vadinamąją minimalią fototoksinę dozę (MPD). Tai rodo, nuo kurio ultravioletinių spindulių poveikio odos su fotosensibilizatoriumi yra paraudimas. Po to, kai psoralenas buvo veiksmingas, pirmojoje sesijoje apšvitinama tik 20-30 procentų MPD, o dozė lėtai padidėja. Visoje UVA fototerapijoje būtina dėvėti specialius akių apsaugos akinius.
